In this article, an ethical analysis of an educational programme on renal replacement therapy options for patients and their social network is presented. The two main spearheads of this approach are: (1) offering an educational programme on all renal replacement therapy options ahead of treatment requirement and (2) a home-based approach involving the family and friends of the patient. Arguments are offered for the ethical justification of this approach by considering the viewpoint of the various stakeholders involved. Finally, reflecting on (...) these ethical considerations, essential conditions for carrying out such a programme are outlined. 'Uncertainty and chance' is a subject with a broad span, in that there is no academic discipline or walk of life that is not beset by uncertainty and chance. In this book a range of approaches is represented by authors from varied disciplines: natural sciences, mathematics, social sciences and medical sciences. At one extreme, this volume is concerned with the foundations of probability. At the other extreme, we have scholars who acknowledge the concept of chance and uncertainty but do not (...) cope with it by means of systematic measurement or quantative analysis. (shrink)

One area of language research that has received a great deal of attention, both theoretical and empirical, is the use of anaphoric expressions. Such expressions can be thought of as serving two functions: the primary function is to refer back to a referent from previous discourse, and the secondary, but no less important, function is to help provide discourse coherence and structure. Third person pronouns such as he or she are anaphoric expressions par excellence, but fuller anaphoric expressions, including demonstrative (...) and definite noun phrases (NPs) such as that woman and the woman are also used in natural discourse. In this chapter we shall focus primarily on issues concerning definite NP anaphor resolution, and in particular we shall examine the interaction of two factors that are related to the identification of antecedents: the focus status of the antecedent and the semantic relationship between the antecedent and the anaphor (including semantic overlap). After presenting these factors, we will discuss one particular approach to anaphor resolution, Almor's (1999) Informational Load Hypothesis (ILH), and present three experiments that examined the findings presented in Almor (1999). The results of these experiments will lead us to consider in more detail the secondary, discourse-structuring function of anaphoric expressions. (shrink)

Objective: To review the principles and practice of the use of conscious sedation for IVF.Design: The pertinent literature was reviewed and recommendations are provided.Result(s): Conscious sedation appears to be the most commonly used method of pain relief for transvaginal retrieval of oocytes. Conscious sedation does not require the presence of an anesthesiologist and can be done in freestanding clinics. Agents commonly used include opioids in combination with benzodiazepines. This combination minimizes pain, decreases anxiety, and provides sedation and some amnesia Adjuvants (...) such as promethazine and hydroxyzine can also be used but often are not needed. Conscious sedation is well tolerated by patients and does not require highly specialized equipment. However, there are specific safeguards that should be followed. Only a few toxicity studies have been performed, but they are reassuring because they have not found significant effects on fertilization or cleavage.Conclusion(s): Conscious sedation appears to be a safe and cost-effective method of providing analgesia and anesthesia for transvaginal retrieval of oocytes. This paper introduces the logic _Q__L__E__T_ _F_, a quantified extension of the logic of evidence and truth _L__E__T_ _F_, together with a corresponding sound and complete first-order non-deterministic valuation semantics. _L__E__T_ _F_ is a paraconsistent and paracomplete sentential logic that extends the logic of first-degree entailment (_FDE_) with a classicality operator ∘ and a non-classicality operator ∙, dual to each other: while ∘_A_ entails that _A_ behaves classically, ∙_A_ follows from _A_’s violating some classically valid inferences. The semantics of _Q__L__E__T_ (...) _F_ combines structures that interpret negated predicates in terms of anti-extensions with first-order non-deterministic valuations, and completeness is obtained through a generalization of Henkin’s method. By providing sound and complete semantics for first-order extensions of _FDE_, _K3_, and _LP_, we show how these tools, which we call here the method of _anti-extensions + valuations_, can be naturally applied to a number of non-classical logics. Decisions related to animal welfare standards depend on farmer’s multiple goals and values and are constrained by a wide range of external and internal forces. The aim of this paper is twofold, i.e., to develop a theoretical framework for farmers’ AW decisions that incorporates farmers’ goals, use and non-use values and to present an approach to empirically implement the theoretical framework. The farmer as a head of the farm household makes choices regarding production to maximize the utility of the household. (...) The overall utility of the farmer is determined by his multiple objectives. For the analysis of multi-objective problems, the multiple criteria decision - making paradigm provides an appropriate theoretical framework. However, theories from the field of social-psychology are needed to facilitate the identification of all relevant aspects in the decision making. The practical use of the conceptual framework is demonstrated using a simple numerical application of a multi-objective programming model. Two workshops were devoted to examining the scientific consistency and the practical usefulness of the approach. Implementing this approach will increase knowledge of the main factors and barriers that determine farmers’ decisions with regard to AW standards. This knowledge is relevant during the development of new AW concepts that aims to supply products that comply with above-legal AW standards for middle-market segments. (shrink)

This book contains three essays: "The Mask and the Face: The Perception of Physiognomic Likeness in Life and Art" by Gombrich, the renowned art historian and critic; "The Representation of Things and People" by psychologist, Julian Hochberg; and "How Do Pictures Represent" by philosopher, Max Black. The book is based upon lectures delivered in the Johns Hopkins 1970 Thalheimer Lectures, where, taking off from the question "how there can be an underlying identity in the manifold and changing facial expression of (...) a single individual," there is an interdisciplinary attempt at clarifying the problem of representation. Gombrich’s central thesis is that the key to artistic representation is empathy and projection, which is guided by the interlocking display of the permanent and mobile features of the object represented. Perceptual activity and empathy rely more on the muscular imitation than on passive visual reception. He holds that what is singled out as the likeness-factor uniting the permanent and mobile features in, for example, the photograph of the four-year old Lord Russell and such factors in the ninety-year old Russell is the "general tonus, the melody of transition from given ranges of relaxations to forms of tenseness." Hochberg’s essay spells out the position that perception is purposive behavior, wherein the purpose is the information sought and behavior is the "succession of glances in different directions." Holding that perceptual activity is grounded in expectations, he lays aside Gombrich’s muscularity-thesis in favor of a learned expectation of feature characteristics. Black’s essay is a conceptual analysis of "depiction," or more precisely, of "P displays a subject S if and only if R, where R ‘will constitute the necessary and sufficient condition for P displaying S'." The essay, which proceeds in a Wittgensteinian Investigations-type fashion ends where the reader would hope it begin. He concludes that the problem can only be adequately answered by moving from logical investigation to the world of the artisan and art lover. To arrive at this conclusion he debunks six candidates that claim to meet the depiction conditions: causal history, selective information, intention, mimesis, resemblance, and "looking-like." At best, depiction may be considered a "cluster" concept of all six. Further determination, he holds, requires knowledge of the purpose of a particular depiction, and this takes us out of logic and into art; and so Black stops, unfortunately.—W. A. F. (shrink)

Introduction Stored human samples and the establishment of biobanks are increasing in the world. Along with this there are the questions of ethics that arise such as the correct method of obtaining informed consent for research on stored samples and the policies involved in collaborative research using collected samples. This study is an attempt to evaluate the researchers, academics and policy makers' views on these ethical aspects. Methods This was an anonymised study involving a Sri Lankan population of researchers, ethics (...) committee members, and policy makers. A self administered questionnaire was utilised as the study instrument. The questionnaire captured four major areas of interest: demographic characteristics of respondents, their attitudes on informed consent policy, their opinion on rights of collaborating researchers, their attitudes on dealing with international differences in regulatory frameworks. Results The study included 55 responders with 40/55 agreeing that donors should receive the option of giving informed consent for future research, with 31/55 considering multiple- type consent options most appropriate. Regarding the issue of shared samples in collaborative research majority agreed that source country ethics review committee approval was necessary 53/55. Conclusion The study concludes that sample donors should be given the option of giving advance consent to unspecified future research provided that future research is approved by an ethics committee. In collaborative research, it is necessary to involve ethics committees from donor countries in the research approval process. (shrink)
